Here are the steps on how to apply for a
Nursing Licensure Examination/ Board Exam:
1. Secure PRC Application Form from the PRC Information Desk or Public Assistance Counter. You may also download the form at
www.prc.gov.ph

2. Fill up the PRC Application Form and proceed to Processing Counters together with the following requirements:
FOR FIRST TIMERS:
a. Transcript of Records (Original and Photo Copies) with degree/course, Special Order, date of graduation, scanned picture and with a remark of "FOR BOARD EXAM PURPOSES ONLY".
NOTE:
* Graduates from government schools/ institutions or programs accredited by the recognized accrediting agencies under FAAP are exempted to S.O. (Special Order).
* Graduates of new schools/ degree/ programs must submit a School Recognition or Permit to Operate.
b. Notarized OR (Operating Room) and DR (Delivery Room) Cases: (5) Five Major Scrubs, (5) Five Minor Scrubs, (5) Five Handled Delivery, (5) Five Assisted Delivery and (5) Five Cord Dressing.
c. Notarized of Summary of Related Learning Experience (RLE).
d. NSO- Birth Certificate (Original and Photo Copies)
e. Marriage Contract issued by NSO (Original and Photo Copies) *For married applicant only
f. Four (4x) pieces of
Passport size picutre with white background and complete name tag at the bottom of the picture.
g. One (1x) piece
mailing envelope with metered postage stamp.
h. Cedula (Current Community tax Certificate)
FOR REPEATERS:
a. Transcript of Records (Original and Photo Copies) with degree/course, Special Order, date of graduation, scanned picture and with a remark of "FOR BOARD EXAM PURPOSES ONLY".
* Graduates from government schools/ institutions or programs accredited by the recognized accrediting agencies under FAAP are exempted to S.O. (Special Order).
* Graduates of new schools/ degree/ programs must submit a School Recognition or Permit to Operate.
c. NSO- Birth Certificate (Original and Photo Copies)

Secure the
Permanent Examination and Registration Record Card (PERRC) and
Notice of Admission (NOA).
* These two forms will be given at the Processing Counters.
4.
Pay the Examination Fee at any Cashier Windows. Don't forget to get the OR (Official Receipt) after payment.
- Baccalaureate Degree - P900.00
- Non- Baccalaureate Degree - P600.00
- Removal - P400.00
5. Fill up all PRC Forms and copy the application number, OR (Official Receipt) Number and the date on your OR to the Notice of Admission (NOA)
6. Pay for mailing envelope with metered documentary stamps at the CSC or PRC Coop Store.
7. Proceed to the Issuance Window or other designated counters for the final review of qualifications, documents and issuance of Notice of Admission (NOA)
8. Keep the Notice of Admission together with the Official Receipt and bring them on the day of your exam.

9. The
schoold and room assignment will be posted two (2) to three (3) days before the exam. Bring the NOA Form when verifying the assigned school and room for you. You also check it Online.
