* Graduates from government schools/ institutions or programs accredited by the recognized accrediting agencies under FAAP are exempted to S.O. (Special Order).
* Graduates of new schools/ degree/ programs must submit a School Recognition or Permit to Operate.
b. Notarized OR (Operating Room) and DR (Delivery Room) Cases: (5) Five Major Scrubs, (5) Five Minor Scrubs, (5) Five Handled Delivery, (5) Five Assisted Delivery and (5) Five Cord Dressing.
c. Notarized of Summary of Related Learning Experience (RLE).
d. NSO- Birth Certificate (Original and Photo Copies)
e. Marriage Contract issued by NSO (Original and Photo Copies) *For married applicant only
f. Four (4x) pieces of Passport size picutre with white background and complete name tag at the bottom of the picture.
g. One (1x) piece mailing envelope with metered postage stamp.
h. Cedula (Current Community tax Certificate)
